Supervisor Referrals

MANAGER/SUPERVISOR REFERRAL PROCESS

One of the primary strengths of the American Behavioral Employee Assistance Program (EAP) is to partner with your company in your efforts to seek solutions and resolve conflicts affecting your employee's job responsibilities. Our qualified staff can help managers/supervisors identify and respond to declining work performance or behavioral problems. The following information will allow us to help you and your employee through a smooth and simple referral process.

· FIRST- call Amy Colvin, with the Manager Support Center at (205) 868-9613 or (800) 925-5327.

· Provide EAP with specific work performance problems and conditions of the referral as well as any company specific policy/procedures, contracts for continued employment or substance abuse protocol (if applicable).

· Inform EAP if the employee is on probation, suspended, or continuing on the job.

· After you have explained to the employee the reasons for the mandated referral, direct the employee to call within 24-48 hours to schedule his/her initial assessment. Bridgett Bowman is the contact person and can be called on her confidential direct line at (205) 868-9613, or any EAP number.

· The manager/supervisor will be notified if the employee has or has not made the call to EAP within the time frame allowed.

· A HIPAA release of information form will be signed by the employee on the first visit so the manager/supervisor can be informed of the assessment outcome, weekly progress reports and recommendations.

· A Progress Report is prepared after each appointment, measuring the degree of motivation and the level of compliance. Supervisors will be provided these reports via choice of fax, mail or verbally by telephone.

· After an appropriate number of EAP sessions, the final Summary and Recommendations Report will also be provided to the manager/supervisor.

Manager/Supervisor Referrals provide an opportunity to help employees remain productive and for management to continue their on-going support. The Employee Assistance Program ensures the privacy and confidentiality of the employee's records and personal information.

· Direct the employee to call EAP (Amy) within 24-48 hours.

· Allow initial EAP assessment during regular work hours.

· Continue to observe the employee and inform EAP of any further declining work performance or behavioral problems.
