If a client referred through an American Behavioral plan, a referral will be faxed, emailed or called into the provider's office, according to their preference. The following conditions apply for payment and additional authorizations.
American Behavioral can accept paper and electronic claims.
Managed Behavioral Health & Substance Abuse Referrals
- American Behavioral issued "Managed Behavioral Health Fee Schedules applies for payment
- See the "Authorization" section for any pre-authorization requirements.
- Members co-pay will have a co-pay, if applicable to their plan.
- File on a CMS 1500

All BlueCard Claims with dates of service after December 31st, 2009 should be sent to Managed Health Care Administration (MHCA) at 1-866-334-6422
Employee Assistance Program Referrals
- American Behavioral issued "EAP Fee Schedules applies for payment
- Pre-Authorization is needed.
- Authorization numbers are required.
- Members will NOT have a co-pay
- File on a CMS 1500
- Use V Code V71.09 with no diagnosis or condition on Axis I
